After
completion of a grant, the recipient is required to fill out an "End of
Grant" form. This form is provided here for electronic transmission to
the Idaho Community Foundation.
If you are having trouble viewing or downloading the form, please ensure
you are updated to the latest version of Adobe Reader.

Note: The form must be filled out
completely to be eligible for future grants. Writing "See
Attached" in the fields and including other correspondence and/or
documentation is not an acceptable substitution for fully completing all
pertinent sections of the form.
