After
completion of a grant, the recipient is required to fill out an "End of
Grant" form. This form is provided on this website for electronic
transmission to the Idaho Community Foundation, or for completion and
transmission via FAX, email, US Mail, or delivery.
Note: The form must be filled out
completely to be eligible for future grants. Writing "See
Attached" in the fields and including other correspondence and/or
documentation is not an acceptable substitution for fully completing all
pertinent sections of the form.
There are online forms you may use to complete your End Of Grant
Report:
1. A PDF form, which may be completed while online, then
automatically submitted via clickable button. Alternatively, you
may complete the form online and print it to your personal printer, then
mail or deliver to the Foundation. You may also be able to Save
the completed form to your computer, then attach to an email to ICF
directed to edavis@idcomfdn.org.

2. An Electronic form, which may be used as an alternative to the
PDF form, and which will automatically submit your form data to ICF upon
completion and clicking the Submit button at the bottom of the form.
You will not need Adobe Reader installed, or any email program
installed, to successfully complete and send this form to us.

3. A Microsoft Word version of the End of Grant Report
form, which may be downloaded by clicking
here. If you use this
version, please complete and email, mail or deliver it to ICF. To

Please note that it is very important to complete and return the End
of Grant Report in the time frame set forth in your Grant Agreement.
Please allow sufficient time to accomplish this before the appropriate
